This Community Health Worker role is a critical hire for the company. We are looking for someone that has big ambitions and can both roll up their sleeves in the short term and can build and scale a team around them. Join a behavioral health care effort dedicated to providing support via an innovative new patient experience that leverages the latest advancements in AI and behavioral economics.
Nuna has been at the forefront of value-based care for over 5 years, and as part of that work has identified a new AI-based offering that supercharges the relationship between physicians and patients by helping patients have healthier behaviors that work with their caregivers. Nuna is moving very rapidly to bring this product to market, and already has several large scale deployments committed for 2024.
The ideal candidate will be a highly effective Community Health Worker who currently lives and has worked with patients and communities in Florida, specifically Tampa, Orlando, and Tallahassee, and is passionate about improving patient outcomes via use of technology. This role demands a highly motivated and empathetic individual, capable of multitasking in a dynamic environment and committed to improving the health and wellbeing of local communities.
YOUR OPPORTUNITIES
Patient Enrollment and Insight Sharing
Enrolling and onboarding patients into care management programs supported by the Nuna app
Helping patients set up remote health monitoring devices (BP monitor, CGM, etc.) and integrating it with the Nuna app
Cataloging patient enrollment information into a centralized portal
Following up with patients who are enrolled into programs to maintain engagement
Documenting and sharing patient insights and feedback with technical team members to help improve the patient experience
Educational and Supportive Responsibilities
Educating members on preventative care, particularly the importance of medication adherence and healthy lifestyle
Virtually or in-person support for members' health needs, including short-term interventions and collaborative care management
Conducting proactive outreach and maintaining engagement with members, especially those with high needs, to help them achieve their health goals
Documenting member interactions and organizing this information efficiently
Referral and Assessment Responsibilities
Making necessary referrals to the care team for additional medical or social resources
Conducting thorough needs assessments with patients in various settings and assisting with health-related social issues
Helping patients set and reach health goals, addressing social determinants of health, and assisting in patient coaching
Coordination and Community Outreach Responsibilities
Creating and updating a directory of local community resources
Managing relationships with healthcare facility providers and working closely with the care team
Expanding knowledge of community resources and building relationships with these organizations
Serving as a liaison between services, aiding in care coordination, and advocating for clients
Attending community events for outreach and education
Technology and Documentation Skills Responsibilities
Teaching patients how to use the Nuna app and providing critical feedback from the field to the Nuna product team
Navigating various technology systems for detailed and accurate documentation of patient encounters
Supporting both in-person and virtual patient interactions through phone or video visits
QUALIFICATIONS
Required Qualifications
Bilingual - Proficiency with English and Spanish (some familiarly with Creole is nice to have)
A.A.S. Degree, Advance Certificate or equivalent experience- Community Health Worker (CHW) Program
State of Florida resident
Proficiency in using technology for remote support and comfortable with virtual communication platforms
Strong problem-solving skills with a creative approach to healthcare challenges
Adaptability to change, excellent organizational skills, ability to work independently
Strong communication skills, cultural competency, and a commitment to serving diverse and underserved populations
Valid driver’s license and insured vehicle access
Preferred Qualifications
Community Health Worker certification
Experience with home visits, managed care patients, and roles in customer or client service
Knowledge of FL ACA populations
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The expected salary range for this position is $72,000 to $85,500. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, and skillset.
